Quick Answer: Read the active ingredient, not the brand. Unisom SleepTabs and ZzzQuil Ultra contain doxylamine succinate 25 mg; original ZzzQuil (liquid and LiquiCaps) and Unisom SleepGels contain diphenhydramine 50 mg, the same drug as Benadryl. Both are sedating antihistamines labeled for occasional sleeplessness in people 12 and older, one dose at bedtime. Doxylamine tends to last longer, so it is more likely to leave you groggy the next morning; diphenhydramine wears off a bit sooner. Neither is meant for nightly long-term use, and both are best avoided by adults 65+ because of anticholinergic side effects. If sleeplessness lasts more than 2 weeks, the labels say to see a doctor.

Why “Unisom vs ZzzQuil” Is Really Doxylamine vs Diphenhydramine
Both brands sell more than one formula, and they cross over. Unisom’s blue SleepTabs are doxylamine, while its SleepGels are diphenhydramine. Vicks’ original ZzzQuil is diphenhydramine, but ZzzQuil Ultra tablets are doxylamine. So two boxes with different brand names can hold the same drug, and two boxes from the same brand can hold different drugs. Check the “Active ingredient” line at the top of the Drug Facts panel. ZzzQuil liquid also contains alcohol (10% on the label we reviewed), which matters if you avoid alcohol.
None of these products contain melatonin unless the label says so. Vicks also sells melatonin gummies under the ZzzQuil name, which are dietary supplements, not drugs. For how melatonin compares, see melatonin vs magnesium for sleep.
How They Work
Doxylamine and diphenhydramine both block histamine H1 receptors in the brain. Histamine helps keep you awake, so blocking it causes drowsiness. Both also block acetylcholine (anticholinergic effects), which causes dry mouth, constipation, blurred vision and urinary retention, and in older adults can cause confusion. Tolerance to the sedating effect can develop within days of nightly use, which is one reason the labels limit use to occasional sleeplessness.
Do They Work?
The evidence is modest for both. The American Academy of Sleep Medicine’s clinical practice guideline on drugs for chronic insomnia suggests clinicians not use diphenhydramine, because trials showed only small improvements in sleep onset and total sleep that did not clearly outweigh harms. Doxylamine has even fewer modern sleep trials. For an occasional bad night (jet lag, a stressful week), many people do fall asleep faster. For insomnia that lasts weeks, cognitive behavioral therapy for insomnia (CBT-I) is the recommended first-line treatment.
Next-Day Grogginess
This is the biggest practical difference. Doxylamine is cleared more slowly, so more of the drug can still be on board when your alarm goes off, which many users describe as a “hangover.” The Unisom SleepTabs label explicitly warns not to take it unless you have time for a full night’s sleep, and not to drive or operate machinery. Diphenhydramine wears off somewhat faster, but driving-simulator research has shown that it can impair performance as much as or more than alcohol, and people often don’t feel how impaired they are. With either, give yourself 7–8 hours in bed and don’t drive if you feel foggy.
Older Adults: A Real Reason to Avoid Both
The American Geriatrics Society Beers Criteria list first-generation antihistamines, including diphenhydramine and doxylamine, as medicines to avoid in adults 65 and older because of confusion, falls, constipation and urinary retention. Long-term heavy use of strongly anticholinergic drugs has also been associated with a higher risk of dementia in observational studies, though cause and effect isn’t proven. If you’re over 65, talk to your doctor about other options.
Interactions and Doubling Up
- “PM” pain relievers: Tylenol PM and Advil PM already contain diphenhydramine. Taking them with ZzzQuil doubles the dose. See Advil PM vs Tylenol PM.
- NyQuil: NyQuil Cold & Flu contains doxylamine. Don’t take it with Unisom SleepTabs or ZzzQuil Ultra. See DayQuil vs NyQuil.
- Allergy pills: Benadryl is diphenhydramine. Second-generation antihistamines such as cetirizine are less sedating but still add up.
- Alcohol, opioids, benzodiazepines, other sleep drugs, some antidepressants: additive sedation and breathing risk. Ask a pharmacist.
Pregnancy and Breastfeeding
Both labels say to ask a health professional. Doxylamine combined with pyridoxine (vitamin B6) is an FDA-approved prescription treatment for nausea and vomiting of pregnancy, so its pregnancy safety data are relatively extensive, but that doesn’t make an OTC sleep tablet automatically appropriate. Antihistamines can pass into breast milk and may make a nursing infant drowsy or irritable.
Who Should Choose What
- Trouble falling asleep, need to be sharp early: a diphenhydramine product (ZzzQuil, Unisom SleepGels), taken only on a night with a full sleep window.
- Waking in the night, can sleep in: doxylamine (Unisom SleepTabs, ZzzQuil Ultra) lasts longer, at the cost of more morning grogginess.
- 65 or older, glaucoma, enlarged prostate, or breathing disease: avoid both unless your doctor says otherwise.
- Need help most nights: skip OTC antihistamines and ask about CBT-I and screening for sleep apnea, depression, anxiety or restless legs. Our sleep supplements guide covers non-drug options.
When to Talk to Your Doctor or Pharmacist
- Sleeplessness lasts more than 2 weeks (both labels say this; insomnia can be a symptom of an underlying illness).
- You snore loudly, stop breathing in sleep, or wake unrefreshed: possible sleep apnea, which sedatives can worsen.
- You are 65 or older, pregnant or breastfeeding, or have glaucoma, an enlarged prostate, asthma or COPD.
- You take antidepressants, opioids, benzodiazepines, other sleep medicines, or several prescriptions.
- You feel confused, have trouble urinating, or a racing heart after a dose.
Frequently Asked Questions
Is Unisom or ZzzQuil stronger?
It depends on the product, not the brand. Unisom SleepTabs and ZzzQuil Ultra contain doxylamine 25 mg, which lasts longer. Original ZzzQuil and Unisom SleepGels contain diphenhydramine 50 mg. There is no good evidence that one ingredient is more effective, but doxylamine is more likely to cause next-morning grogginess.
Is ZzzQuil the same as Benadryl?
Original ZzzQuil liquid and LiquiCaps contain diphenhydramine, the same active ingredient as Benadryl, at a 50 mg bedtime dose. ZzzQuil Ultra is different: it contains doxylamine.
Can I take Unisom every night?
The labels are for occasional sleeplessness and say to stop and see a doctor if it lasts more than 2 weeks. Tolerance to the sedating effect can develop quickly, and nightly use adds anticholinergic side effects. For ongoing insomnia, CBT-I is the recommended first-line treatment.
Can I take Unisom and ZzzQuil together?
No. Both are sedating antihistamines, and combining them increases drowsiness, confusion and other side effects without a clear benefit. Also avoid combining either with NyQuil, Tylenol PM, Advil PM or Benadryl.
Why do I feel hungover after Unisom?
Doxylamine, the ingredient in Unisom SleepTabs, is cleared slowly, so some is still active the next morning. The label warns to take it only when you have time for a full night’s sleep and not to drive or operate machinery.
Are OTC sleep aids safe for older adults?
The American Geriatrics Society Beers Criteria recommend that adults 65 and older avoid first-generation antihistamines such as diphenhydramine and doxylamine because of confusion, falls, constipation and urinary retention. Talk to your doctor about alternatives.
